What the patient PDF report contains
Patients can export a report for any date range. Depending on which trackers they use, it can include:
- Symptom logs with dates and severity ratings, plus mood, energy and sleep quality
- Chronic pain and CRPS diary entries — severity, location, pain type, triggers and relief methods
- Asthma logs — peak flow readings, symptoms and reliever use
- Seizure diary entries — seizure type, duration, warning signs and whether rescue medication was taken
- Current medications with doses and frequencies, including start/stop dates
- Upcoming and past appointments, and care-team contact details
The report simply organises what the patient has recorded. It is not a diagnosis, a score or a clinical recommendation.
How patients bring it to you
Patients generate the PDF in the app and bring it printed or on their phone. Many email it to the clinic ahead of a consult. Because every entry is dated, it gives you a between-visits history of symptoms, medications and flare-ups rather than a two-week recollection.
What WellNest doesn't do
WellNest makes no clinical outcome claims. It does not diagnose, treat, predict or monitor any condition, and it does not alert you or anyone else to a patient's data. It is a record-keeping tool — clinical decisions stay with you and your patient.
